INFORMATIVA SUI RISCHI E RESPONSABILITA’E RELATIVA ACCETTAZIONE (EU Version) Si prega di leggere attentamente e compilare gli spazi lasciati in bianco prima di sottoscrivere La presente costituisce una dichiarazione volta a fornire informazioni sui rischi dell’immersione in apnea (skin diving) e dell’immersione con autorspiratore (scuba diving). La presente dichiarazione inoltre enuncia le ipotesi in cui i rischi connessi all’attività d’immersione sono da ritenersi a Suo esclusivo carico.*
2. La sottoscrizione di tale documento è richiesta a riprova del fatto che lo stesso è stato a Lei consegnato e da Lei compreso. E’ pertanto fondamentale che Lei legga attentamente il contenuto di tale dichiarazione prima di sottoscriverla, in caso di dubbi circa il contenuto della presente dichiarazione, La invitiamo a discuterne direttamente con il Suo istruttore. Nel caso di minorenni il presente modulo dovrà esser sottoscritto da un genitore, ovvero da un tutore.*
3. ATTENZIONE L’immersione in apnea (skin diving ) e quella con autorspiratore (scuba diving) presentano rischi intrinsechi che possano causare gravi lesioni o morte.L’immersione con aria compressa eo con miscele respiratorie denominate Nitrox e Trimix può comportare a titolo esemplificativo i seguenti rischi: possano insorgere malattia da decompression, embolia o altre complicazioni che possano richiedere cure in camera iperbarica.*
4. Le immersioni, necessarie per la preparazione e l’ottenimento dell’attestato, potranno essere condotta in un luogo lontano, sia per tempo che per distanza, da una camera iperbarica. L’immersione in apnea e con autorspiratore sono attività fisicamente faticose e il presente programma di immersione implica un grande sforzo fisico. E’ pertanto necessario fornire reali e veritiere informazioni sul proprio quadro medico ai professionisti/istruttori nonchè alla struttura che offre tale programma.*
5. ACCETTAZIONE DEL RISCHIO Io sottoscritto comprendo ed accetto che nè gli istruttori che gestiscono questo programma, nè la struttura presso cui questo programma viene svolto, nè UTD Scuba Diving, LLC, nè UTD Europe, nè UTD Italia, nè le loro società affiliate o controllate, nè alcuno dei loro rispettivi dipendenti, funzionari, agenti,o cessionary, possano in alcun modo essere ritenuti responsabili per l’eventuale morte, lesioni o altre perdite da me subite, che siano conseguenza della mia condotta ovvero per altri problemi derivanti da eventi o circostanze dovuti a mia negligenza eo colpa. In assenza di negligenza eo colpa o qualsivoglia altra violazione del dovere di diligenza da parte degli istruttori che gestiscono questo programma, nonchè della struttura presso cui tale programma è gestito, di UTD Scuba Diving, LLC, UTD Europe, UTD Italia e di parti tutti I soggetti sopra indicate comprendo e accetto che tutti I rischi connessi a tale programma sono a mio esclusivo carico.*

If the participant is under the age of 18, then the parent or guardian must PRINT AND SIGN this agreement and agree to be legally bound by it and furthermore be legally responsible for the minor participant, including being responsible for all damages, injury or death which may occur as a result of the minor’s participation in diving activities. The parent or guardian hereby agrees to be fully responsible to the “Released Parties” for any damage, injury or death caused by the minor, including actions brought by the minor, for any damages whatsoever.
